**If you have Microsoft Word (2003 or newer) use the list above and click download beside the form you wish to print. (The "Turning the Tide" document includes the Medical Consent and Release Form)
If you do not have Microsoft Word, click on one of the links to the right, and these will take you to a picture. Right click and choose "Save As" to save the picture or choose File>Print at the top. (The "Turning the Tide" document only contains information and does not include the Medical Consent and Release Form)
**Printing using Microsoft Word is preferable, seeing as with the pictures, they come out a little small...sorry.
